  • Tattoos: Under 16, tattooing is prohibited except for medical/dental reasons performed by a licensed medical professional. Ages 16–17 may receive tattoos only if a parent/legal guardian is present, both show government ID, and the parent/guardian provides written notarized consent using the Florida Department of Health (DOH) format.

  • Body Piercing: A minor may not be pierced without written notarized consent from a parent/legal guardian. Minors under 16 must also be accompanied by a parent/legal guardian.

What to Bring to the Appointment

  • Government-issued photo ID for the minor and the parent/legal guardian (driver’s license, passport).

  • Proof of relationship (birth certificate, court guardianship documents), as applicable.

  • The completed consent form (tattoo or piercing) signed in front of a notary (or signed and then acknowledged before a notary at the time of notarization).
